The two forms that matter

Once per company · HR or Team Captain

Company registration

Registers your company for the founding season and locks your projected roster size. Includes the authorized-representative signature. Due Wednesday, October 14.

Once per participant · every employee competing

Participant sign-up & waiver

Each employee picks the events they're competing in and completes the liability waiver and media release. Nobody starts an event without this on file.

Sign Up & Sign Waiver

Team Captains: send the participant link to your roster once your company is registered. The front office sends you a running list of who has signed up and who still owes a waiver.
